How to request a record
- Identify the record clearly and state your preferred form of access.
- Explain the right you seek to exercise or protect and why the record is required for that purpose.
- Send the prescribed PAIA Form 2, or a substantially equivalent written request, to the Information Officer.
- Provide proportionate proof of identity or authority only when requested. Do not send unnecessary identity documents.
